PARENTAL CONSENT AGREEMENT

This is to certify that I, as a parent/guardian, have legal responsibility for the participant named herein.  I do consent and agree to his/her release and, for myself, my heirs, assigns, and next of kin.  I release and agree to indemnify and hold harmless Team Bass Tournament Series, tournament officials , sponsors and members from any and all liabilities to minor’s involvement or participation in any Team Bass Tournament Series events.
